Privacy policy
Your privacy
Last updated:
Ditekemena Psychotherapy & Wellness is committed to protecting your personal health information. This policy explains what information I collect, why, how it is protected, and your rights. It is written to comply with Ontario's Personal Health Information Protection Act, 2004 (PHIPA) and the standards of the College of Registered Psychotherapists of Ontario (CRPO). Please review it with the therapist or a lawyer before publishing.
Who is responsible for your information
Emile Yav Kayembe, Registered Psychotherapist (Qualifying), is the health information custodian responsible for the personal health information collected in the course of providing psychotherapy services.
What information is collected
Contact and identifying information (name, date of birth, address, phone, email, emergency contact); information about your health and personal history relevant to therapy; session notes; billing and payment records; and, for virtual sessions, the information needed to confirm your location in Ontario and your identity.
Why it is collected
To provide safe and effective psychotherapy, to communicate with you about appointments, to issue receipts for insurance purposes, to meet professional record-keeping obligations under the CRPO, and to comply with the law.
Clinical supervision
As a Registered Psychotherapist (Qualifying), I practise under clinical supervision. Aspects of our work may be discussed with my clinical supervisor, who is a regulated health professional bound by the same duty of confidentiality. Supervision is part of providing you with quality care, and it is explained in the consent-to-treatment document you sign before therapy begins.
When information may be shared without your consent
Confidentiality has a small number of legal limits. Information may be disclosed without consent when there is a serious risk of harm to you or to another person; when there is a reasonable suspicion that a child is in need of protection; when a regulated health professional or a resident of a long-term care or retirement home is suspected of abuse; when required by a court order or subpoena; or when the CRPO requires records for a quality assurance or complaint process. Wherever possible, only the minimum necessary information is shared.
How your information is protected
Records are kept in a secure, encrypted, PHIPA-compliant practice management system hosted in Canada. Paper documents, if any, are stored in a locked cabinet. Virtual sessions use an encrypted video platform. Access to records is restricted to the therapist and, where applicable, the clinical supervisor.
How long records are kept
In accordance with CRPO standards, clinical records are retained for at least 10 years after the last contact, or for at least 10 years after a client's 18th birthday if the client was a minor, and are then securely destroyed.
Your rights
You have the right to access your record, to request a correction, to withdraw consent for future collection or sharing (subject to legal limits), and to ask questions about how your information is handled. If you have a concern, please raise it with me first. You may also contact the Information and Privacy Commissioner of Ontario at 1-800-387-0073 or ipc.on.ca.
